As of the fourth quarter of 2023, Evergreen Nursing and Rehabilitation Center was owned by Stephen Miller, according to Centers for Medicare and Medicaid Services (CMS). The nursing home, located in Effingham County, has been owned by Stephen Miller since Feb. 28, 2018.
