Newman Rehab and Health Care Center, a 60-bed skilled care center in Newman, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Newman Rehab and Health Care Center was cited with an A violation in October and fined $25,000.
