Cumberland Community Unit School District 77 issued the following announcement on July 16.
There are a few requirements upon entering certain grades in school. Below you will find the school physical and immunization requirements by grade. Please note, athletic physicals will not meet the school physical requirements, but a school physical will work for athletics.
Pre-K: School Physical, Lead Screening, Immunizations
Kindergarten: School Physical, Lead Screening, Vision Exam, Dental Exam, Immunizations
2nd Grade: Dental Exam
6th Grade: School Physical, Dental Exam, Immunizations
9th Grade: School Physical, Immunizations: two doses of Varicella, one dose of the Meningococcal vaccine, and the TDAP vaccine.
12th Grade: Immunizations: two doses of Varicella and two doses of the Meningococcal vaccine (only one if the first one was administered after the age of 16.)
